International Student Health Insurance

All students attending Southern Polytechnic State University on an F1, J1, F2, or J2 visa are required to have the Mandatory Health Insurance with the university. The student’s mandatory insurance will be billed and paid through banner web. The provider of the student health insurance is United Healthcare Student Resources.

You must enroll for the insurance at www.uhcsr.com/usg/ EVERY semester.

If you have your own insurance, you must apply for a waiver before the wavier period ends.

To Enroll and Request an insurance card

  1.  Go to www.uhcsr.com/usg/
  2. Choose Southern Polytechnic State University from the list
  3. Click on “Enrollment Form” in blue
  4. Scroll down and Click “Enrollment Form” again
  5. Enter your Date of Birth and SPSU Student ID, then hit Continue
  6. Update your mailing address, email, and phone number
  7. Check the Box for the current semester and the charge will appear (You will not be charged twice even though it shows a balance)
  8. Check the next Box to elect coverage
  9. Electronically sign your name and hit Submit
  10. To Log in: CLICK HERE

You should now be able to log in and see your account information. You can choose to order a hard copy card be mailed to you in your account, or you can print it out. The request for a card is listed in the middle of your account information once you have logged in.

To waive the insurance

  1. Please go to www.uhcsr.com/usg/
  2. Choose Southern Polytechnic State University from the list
  3. Scroll down and Click on “Waiver Form” in blue
  4. Scroll down and Click “2012-2013 SPSU Spring Waiver Form”
  5. Enter your Date of Birth and SPSU Student ID, then hit Continue
  6. Complete questions for Waiver according to your current insurance policy.

The Waiver process will take approximately 7-10 business days. You will receive an approval or a denial.
