Responsibilities Each Semester
Once you have registered for your courses for the semester, submit a copy of your
class schedule to the Disability Service Provider (DSP) in the ATTIC. At this point
you will be issued a Faculty Accommodations Form (FAF).
You must pick up the FAF and deliver them personally to the faculty member during
the first week of class. The FAF can be presented during the faculty member's office
hours or by setting up an appointment. This meeting is a wonderful opportunity to
begin communication with the faculty member. At this meeting you can discuss initial
concerns about the course as well as establish your testing preference for the semester.
There are three options for testing. These options are listed below:
- Test with the class and let the professor know if you need extra time to complete
the exam at the end of the exam.
- Test in an established private/separate space within the department.&; You may stop
by the ATTIC the day of testing to check-out a pair of noise canceling headphones.
- Test in the ATTIC. Process defined in Schedule a Test in the ATTIC section.
The student is responsible for communicating and working with the faculty member to identify the best option for their testing needs for that course.
The student must return the FAF to my office after obtaining signatures to be enrolled in accommodations each semester.
Recap of Procedure:
1) Bring your schedule to the DSP ATTIC and pick up the FAF.
2) Take the FAF to the faculty member to identify your access to accommodations.
3) Bring the signed FAF back the the DSP in ATTIC.
Schedule a Test in the ATTIC
Students are encouraged to arrange testing accommodations with the faculty so that
the student has access to the faculty during testing. If arrangements cannot be made,
then the student may utilize the testing facilities in the ATTIC providing that space
is available.
If your professor and you agree that testing shall occur within The ATTIC, schedule
your testing suite as soon as you know the testing time and date with the DSP, you
can stop by the office or e-mail the request. We have limited facilities and proctors,
so advance scheduling is critically important 3 business day notification required.
-Testing hours
Fall/ Spring M-F 8:00AM-5:00PM
Summer M-TH 8:00AM-6:00PM
You MUST take the test at the same time as the class (with moderate adjustment for
extra time, such as starting early or staying later). If you need to take your test
at a time that is different from the class testing, your teacher MUST approve the
change.

Request Textbooks in Alternative Media Format
Requests to provide materials in alternative format (Braille, Recorded, Digital, etc.)
must be made in timely manner. If you are eligible to receive materials in Alternative
Format and wish to place an order please provide our office with the following information:

The ATTIC makes every effort to provide text in an alternative format in a timely manner for students with print disabilities, advanced notification is absolutely essential.
Timely Manner
Students are responsible for requesting accommodations at the beginning of the semester
or well before any deadlines. Students who do not request an accommodation prior to
the deadline may not receive the accommodation. As a general rule, accommodations
will not be extended retroactively.
